New Patient Forms
Please print out and complete the new patient paperwork prior to your first appointment.
During your first treatment we will briefly discuss your conditions, health history and review your new patient paperwork. The discussion is followed by a tongue and pulse assessment. Next, the needles are inserted and retained for a period of 20-40 minutes. Most people feel a deep sensation of relaxation and sometimes fall asleep. Prior to your treatment, eat a light snack and drink a bit of water. Wear loose, comfortable clothing that allows access up to your knees and elbows.
Stumptown Acupuncture does not accept insurance at this time. Patients who have insurance that covers acupuncture, can receive a receipt to submit to their insurance company.
